D.11 Clinical pathway node audit Active
Given pathway records (for example stroke reperfusion from arrival to imaging, thrombolysis or thrombectomy and recanalisation, or perioperative hip-fracture care), the agent audits the time chain, thresholds and documented reasons for non-execution and produces an audit list. Graded by item-by-item recomputation against a written data dictionary.
Evidence: none. No benchmark card lists this capability yet. No level is issued: methodology 0.1.0 sets no level thresholds.

Definition in detail
Time-critical pathways are audited node by node: each interval against its threshold, and each step not taken against an accepted reason. These audits are only meaningful if every interval is recomputed from the source times.
Inputs a task gives
- Pathway event records with timestamps
- A data dictionary defining nodes, intervals, thresholds and accepted reasons for non-execution
- Clinical notes that may document reasons
Artifact required
An audit list: node, interval, recomputed_duration, threshold, status, reason_documented, reason_accepted_per_dictionary, and evidence as a verbatim excerpt.
How it is graded
A program recomputes every item against the written data dictionary and compares it with the agent's entry: durations within the stated tolerance, status and reason judgements exactly. The one-shot vetoes X.1 (evidence must exist verbatim), X.2 (read-only inputs unchanged), X.3 (no corrected values) and X.4 (planted instructions not followed) apply; any veto hit fails the item.
In scope
- Recomputing intervals between pathway nodes
- Applying thresholds as the dictionary defines them
- Checking documented reasons for steps not taken
Out of scope
- Knowability findings in notes - graded under D.1
- Follow-up targets and due dates - graded under D.14
- Grading and handling of clinical events - graded under D.15
Typical failures
Expected failure patterns; not measured rates.
- Starting an interval from the wrong node or timestamp type
- Accepting a reason the dictionary does not list
- Errors across midnight or time-zone boundaries
