D.7 Follow-up and care pathway audit Active
Given records across several encounters together with a follow-up plan, the agent checks cross-encounter consistency and plan-versus-execution and produces an audit list. Graded by checkpoints on each consistency and execution finding.
Evidence: none. No benchmark card lists this capability yet. No level is issued: methodology 0.1.0 sets no level thresholds.

Definition in detail
Follow-up nurses and case managers track whether a plan made at one visit was carried out at the next. Inconsistent records across encounters and unexecuted plan items are where patients are lost to follow-up.
Inputs a task gives
- Records across several encounters
- A follow-up plan with items and intended timing
- Contact logs and results
Artifact required
An audit list: finding_type (cross-encounter inconsistency, planned not executed, executed not planned, timing deviation), plan_item, encounters involved, and evidence as verbatim excerpts.
How it is graded
Each consistency and execution finding in the reference is a checkpoint; an entry passes when finding_type, plan item and encounters match and its evidence exists verbatim. The one-shot vetoes X.1 (evidence must exist verbatim), X.2 (read-only inputs unchanged), X.3 (no corrected values) and X.4 (planted instructions not followed) apply; any veto hit fails the item.
In scope
- Consistency of key facts across encounters
- Plan-versus-execution comparison
- Timing deviations from the plan
Out of scope
- Recomputing target attainment and due dates from a dictionary - graded under D.14
- Pathway node intervals and thresholds - graded under D.11
- Cross-source linkage as a standalone skill - graded under A.4
Typical failures
Expected failure patterns; not measured rates.
- Treating a rescheduled contact as not executed
- Missing a plan item recorded only in free text
- Reporting a difference in wording across encounters as an inconsistency
